CPOE Test Name

Follow instructions on Transfusion Tag and Transfusion Reaction Physician Report form. Blood Bank will reflex an order for transfusion reaction workup.

Specimen Requirements

1 Pink top (EDTA), 1 lavender (EDTA), 1 light green tubes and first urine passed post-reaction.

 

Samples require name, MRN, date and phlebotomist initials/Cerner ID.

Storage and Transport

Send to Blood Bank. Hand deliver immediately to a technologist the posttransfusion specimens collected for Blood Bank, the returned blood component bag with intact administration set and a completed Transfusion Reaction Physician Report form signed by physician for the transfusion reaction workup.

Causes for Rejection

Improperly labeled tube, improperly collected tube (try to avoid hemolysis of the red cells during blood specimen collection), or if a Physician Transfusion Reaction Report form is not signed by the physician.

Interpretation

Significant Blood Bank findings would include the following:

Clerical checks revealed a significant clerical error. Posttransfusion reaction serum specimen revealed hemolysis, and pretransfusion reaction specimen did not. Post transfusion reaction DAT test is positive, and the pretransfusion reaction DAT test is negative (further testing would be required to identify the cause). Repeat pretransfusion specimen or the posttransfusion specimen revealed a positive antibody screen result, or repeat pretransfusion crossmatches, or posttransfusion reaction crossmatches were positive indicating significant findings. Such findings would require additional testing to identify cause.

Test Consists

Immediate examination of all clerical work for errors, examination of pretransfusion and posttransfusion reaction serum for hemolysis. ABO, Rh, DAT, and Antibody Screen test on pretransfusion and posttransfusion reaction specimens. Crossmatch, Gram stain, culture, and antibody identification if indicated. Additional testing based on preliminary results, type of suspected transfusion reaction, and patient’s condition. Reference Ranges

Evaluation and investigation by the Blood Bank of any signs or symptoms (acute, delayed, investigation of suspected transfusion transmitted diseases, or look back procedures) experienced by a patient in association with a transfusion as a suspected transfusion reaction. All suspected transfusion reactions will be evaluated promptly and to the extent considered appropriate by the Medical Director.
