Specimen Requirements

2.5 mL EDTA blood should be drawn

If testing will not be performed within an hour from draw, the specimen should be centrifuged, plasma removed, and frozen < -20° C.

Reason for Ordering

Investigation of primary aldosteronism (eg, adrenal adenoma/carcinoma and adrenal cortical hyperplasia) and secondary aldosteronism (renovascular disease, salt depletion, potassium loading, cardiac failure with ascites, pregnancy, Bartter syndrome)

Reference Range

18-40 years: 3.2 - 33.2 pg/mL

> 40 years: 2.5 - 45.1 pg/mL

Pediatric ranges do not exist for Renin concentration.

Aldosterone/Renin Concentration Ratio: < 10 ng/dL/pg/mL = Primary aldosteronism is unlikely

Aldosterone/Renin Concentration Raio: ≥ 10 ng/dL/pg/mL = Primary aldosteronism is probable

Interpretative Information

Renin concentrations are impacted by a number of factors (i.e. salt intake, menstrual phase, pregnancy, use of oral contraceptives, posture, etc.) and medications (i.e. diuretics, calcium channel blockers, ACE inhibitors, ARBs, beta-blockers, clonidine, alpha-methyldopa, NSAIDs, and direct renin inhibitors). 

Causes for Rejection

Incorrect tube, insufficient quantity, clotted specimen, grossly hemolyzed specimen
