Specimen Requirements

No sample required unless there is not a current type on file.
Samples require Name, MRN, date and phlebotomist initials/Cerner ID

Storage and Transport

Notify the Blood Bank when ready for product to be thawed. Cryo will not be prepared until verification from the floor that they are ready for preparation and transfusion.

Causes for Rejection

Improper labeling; wrong tube; gross hemolysis; insufficient volume

Interpretation

This is the product order for the Blood Bank to prepare the product. Product has to be thawed and/or pooled. Once prepared, the pooled product expires in 4 hours. The single product expires in 6 hours. A transfusion order must be placed so that nursing can transfuse the component.

Single: 15 minutes
Pooled: 30 minutes
